    This has the basic quality of a good questionnaire, make sure you USE the questionnaire to find out some ideas for your artist/video - you could ask them to tick the most interesting idea for a video story etc...

A good questionnaire with some good questions but you need to USE the questionnaire to find out what your audiences expectations of the genre is and how best to appeal to them - whether that be by conforming to the generic conventions or challenge them . I wouldn't ask what their fav genre is as if it is not the one you are working in then the questionnaire becomes redundant. rather use the questionnaire to probe your intended audience with what they expect ad what they would like. Give them plenty of options to choose from and images etc .
